Clinical presentation
24-year-old footballer with acute posterolateral knee pain and varus laxity following a forceful varus tackle.
Investigation findings (Knee Radiograph & MRI)
AP knee radiograph and coronal PDFS MRI show an avulsed osseous fragment arising from the lateral aspect of the fibular head (lateral to the styloid tip) remaining attached to the conjoined tendon of the biceps femoris and fibular collateral ligament, with prominent lateral fibular head marrow oedema and a complete mid-substance ACL tear.
Final diagnosis
Biceps femoris avulsion fracture
Case discussion
Prompt anatomical fixation of the fibular head avulsion and posterolateral corner repair is essential to prevent subsequent cruciate ligament graft failure.
